Please call the office for scheduling.  After booking an appointment, you will be sent initial patient forms to complete.

Payment in cash, check or credit card is due at each appointment.

Dr. Levin does not accept insurance, however some plans will reimburse a portion of fees paid for out-of-network services.  Please consult with your insurance carrier in advance.

If you are unable to keep your appointment, kindly give 48 business hours notice to avoid charges.

Any appointment missed or cancelled without 48 business hours advance notice will be charged at the full rate, except in the case of an emergency.

Valet parking in the building is available for a fee.  There is two-hour free parking on Harvard Street south of Wilshire Boulevard, and metered parking on Wilshire.
